Care and Support Coordination Centre

The Care and Support Coordination Service is made up of professionals from the psycho-medical-social sector.
Its mission is to assess home care and assistance needs, centralise useful information and organise the services required for optimal care.
This service supports every stage of the process, working closely with the beneficiary, their family and friends, and other professionals involved, to ensure continuity and quality of care tailored to each situation.

Centralisation of requests and mobilisation of internal and external services
Collaboration with family and friends and the doctor
Coordination of various service providers
Organisation of desired services (home-delivered meals, transport, remote monitoring, etc.)
Preparing to return home after a hospital stay, a stay in a nursing home or a rehabilitation centre
Monitoring and reviewing requirements

Upon receipt of a request, a member of the Coordination Centre will contact the beneficiary or their carer to arrange a meeting and assess their needs.

In collaboration with the beneficiary, their family and friends, and their attending physician, a support and care plan is proposed.

The centre works with all psycho-medical-social stakeholders, without exclusivity or obligation. Each beneficiary is therefore free to choose their service providers.

Thanks to a comprehensive overview of the situation and knowledge of the psycho-medical-social sector, our team ensures the implementation of a personalised, multidisciplinary and evolving care and support plan.
